Population Health and Health Equity Academy

Welcome to the temporary home page of the population health and health equity academy.  The academy is managed and delivered by the ICB population health team in partnership with a range of other stakeholders. The academy exists to help embed, promote and support population health approaches within all our work with the overall aim of informing a reduction in health inequalities within our communities.

The academy seeks to do this via a Build, Enable and Connect model:

  • Build – population health capacity within the workforce via training, learning and development
  • Enable – collaboration by bringing people together to share examples of the great population health work that is going on in Lancashire and South Cumbria and build communities of practice where we can inspire, share ideas and learn together what works to drive improvements in health equity.
  • Connect – people to other population health learning opportunities that are happening in our region by signposting and advertising to partner agency training.

Our academy programme is already up and running, examples of current work include a health inequalities clinical leadership programme and an ongoing series of CORE20PLUS5 workshops.

The population health team has partnered with the King’s Fund to deliver a primary care leadership for population health programme with our newly appointed health inequalities clinical leads, clinical directors and our own place based leads. This has involved collaboration with guest speakers locally and nationally and with our colleagues in public health in Lancashire and South Cumbria.

The purpose of the programme is to support participants to develop their understanding, confidence and approach as primary care leaders for population health; deliver impact in a range of areas this year through the selection and implementation of projects; use and scrutinize data; build connections across their community, build relationships with public health colleagues and within the wider system; understand the leadership landscape and have difficult conversations if needed; and to start to work with their communities in more effective ways for this work. Over time it is hoped that this group will become a resource for the local area to support ongoing focus and optimal efforts for improved population health across Lancashire and South Cumbria for the longer term.

Here's what people had to say about the programme:

“I used the circle technique briefly on my staff this morning ….and after half an hour they all said it meant more than anything we had discussed in the meeting.”

“I feel like I have a much better and effective framework to work from in the future.”

“I felt that the sessions were brilliant. It was insightful to learn about the different tools In the ‘art of participatory leadership’ and the easy applicability of these tools."

We ran two workshops regarding core20plus this year. Core20plus5 is the national NHS approach to support integrated care systems to drive a targeted action to reduce health inequity.

Locally, this has resulted in a real focus on improving the overall health of population and our primary aim is to take steps to improve the health of the parts of our population facing the greatest health inequities.

The sessions focused on local context and Core20PLUS and understanding what further opportunities and gaps there are regarding the support and training that we need to address health inequities in our services or programmes, particularly with regard to collaborative working with a range of partners, including public health colleagues.

We are very excited to be co-hosting the CARE programme in Lancashire and South Cumbria in 2023.

Our colleagues at the National Association of Primary Care (NAPC) are bringing their CARE population health programme later this year within Lancashire and South Cumbria. This consists of eight two hour sessions run bi-weekly from April to July with a focus on both population health management and leadership, wellbeing and resilience. The programme is aimed at GPs, practice staff and allied health professionals, clinical and non clinical.

Expressions of interest has now closed but we will be evaluating and running again in future if it is successful.

The Lancashire & South Cumbria Population Health and Health Equity Academy is now launching its series of Best Practice sessions.

This will be a programme of monthly lunchtime virtual meetings aimed at showcasing all the good work relating to population health and health equity that is going on across Lancashire and South Cumbria with the intention of providing an opportunity for practical peer support for others working in this exciting field of practice.

We will be inviting guest speakers to talk about their project - the what, why and the how  – followed by open discussion to ask questions, explore the work further and make links with colleagues with similar interests. We want the meetings to be informal, enthusiastic and packed full of ideas and expertise to help build a momentum for population health and health equity across the region and to establish a vibrant community of practice to support our collective aspiration to drive forward more equitable health outcomes within our communities.

The intended audience is anyone who has an interest in population health and health equity, who is thinking about starting a project or is already working on a project of their own and who may want some guidance and support. These sessions will appeal to NHS colleagues (including those in primary care, acute settings and community services), public health colleagues and staff working in local authorities and the voluntary sector.

Further best practice meetings will be added here soon.

The modules are aimed at anyone with a responsibility or interest in reducing health inequalities – especially those working to deliver services in the ‘5’ clinical priority areas. The modules are free and each module takes around 30 minutes to complete.  They can be accessed via the link below.
