NHS Lancashire and South Cumbria’s post diagnostic support
Commissioned by the ICB, the dementia navigator service delivers support from Alzheimer’s Society and their partner Age UK Lancashire to people living with dementia and their carers.
Dementia brings symptoms that can challenge day to day life, routines and hobbies, community life and relationships, and future planning, amongst so many situations.
You might have questions about what’s available to you; about managing feelings, or helping to stay confident. You might want to know more of what to expect or to benefit from the lived experiences of others. You might want to explore ideas to help you continue responsibilities, hobbies, or staying independent, for as long as possible.
Throughout Lancashire and South Cumbria, Alzheimer’s Society dementia advisers are available for one to one support for talking these things through, finding ideas, problem-solving workarounds, sharing knowledge, or navigating life and services.
Whether you or a loved one have just had a dementia diagnosis, or diagnosis was a while ago and things have changed, Alzheimer’s Society are available.

Age UK Lancashire
The dementia navigator service is a partnership between Alzheimer’s Society and Age UK Lancashire, with Alzheimer’s Society funding Age UK Lancashire’s delivery of a set of cognitive stimulation therapy groups and monthly peer support and activity through dementia hubs within Lancashire.
